EOHHS launched a new Integrated Care Initiative on November 1st 2013 that may significantly change how individuals on Medicaid receive their long term care services. It includes a new managed care component called Rhody Health Options and a care coordination component called Connect Care Choice Community Partners (CCCCP) program, as well as PACE and traditional Medicaid fee-for-service. We have established this page as an information clearinghouse for our members. As this is an ongoing initiative, it will be updated as events warrant.
Phase 2 of the Integrated Care Initiative officially launches on July 1st 2016, in which a new option for individuals on Medicare and Medicaid will be available through Neighborhood Health Plan of RI. The new plan is called Integrity. The CCCCP program has been discontinued, but the traditional Medicare and Medicaid fee-for-service options remain as well as the PACE program. We will be updating our Q&A documents shortly.
LeadingAge RI has developed the following Q&A documents to supplement the information that consumers are receiving from the state. They are intended to assist consumers, families, and long term care providers navigate this change to our long term care system. A separate document has been created for each of our member industries.
